Dr. Kumetz is in-network with most major PPO insurance plans please call the office for information about your specific plan.
We provide courtesy insurance billing for all patients insurance plans and work with each patient to maximize their out-of-network insurance benefits.
Payment is appreciated at the time of service for all co-pays, deductibles and services not covered by insurance. We accept payment via cash, check or major credit card.
Appointments:
Our friendly and efficient office staff will assist you in scheduling an appointment that is most convenient. We respectfully request that you abide by our 24 hour cancellation policy if you are unable to keep your scheduled appointment so that we can accommodate other patients who need to be seen.
Tips for New Patients:
- Schedule your Annual exam for the week after your period.
- Bring a list of all your current medications including dosages.
- Bring your insurance card and photo ID with you.
- Bring copies of your medical records to your appointment, especially abnormal pap smear results.
- If you intend to have your cholesterol checked, schedule a morning appointment and dont eat or drink anything after midnight the evening before.
- If you are pregnant, schedule your first visit for 7-8 weeks after the first day of your last period.
